Our nursing team, on-call physicians, and personal care aides are always readily available to bring you comfort 24 hours, 7 days a week. We will procure and deliver all your medical supplies and equipment and medication directly to you. We also provide spiritual, emotional, and psychological support, along with education and counseling for our patients and their families.

HOSPICE CARE
When you or your loved one has a terminal illness that has reached its end-of-life stage, it brings many challenges to you and your family, such as discomfort, pain, disability, and emotional suffering. Collective Hospice Care's interdisciplinary team of medical, mental health, social work, and other key professionals understand these challenges. We are dedicated to mindfully curating the best plan to give you the warm, loving care and support you need for a more comfortable and dignified final journey.
Alternatively, if you or your loved one is living with a chronic condition or life-limiting illness, you may qualify for Palliative Care. Our interdisciplinary team is dedicated to curating the best plan to address pain management, as well as physical, emotional, and spiritual needs to preserve the quality of live of those entrusted to our care.

DO YOU QUALIFY FOR HOSPICE CARE?
- To determine qualification for Hospice Care, the hospice doctor and the patient’s primary doctor must certify that the patient is terminally ill and has a life expectancy of 6 months or less. After 6 months, patients can continue to get hospice care as long as the hospice medical director or hospice doctor re-certifies (at a face-to-face meeting) that the patient is still terminally ill.
- Palliative care can be pursued upon diagnosis of a chronic illness, during curative treatment and follow-up, or at the end-of-life stage. Start of care may begin at the discretion of the physician and patient at any time and at any stage of a chronic or terminal illness.
